Job Details
We are seeking compassionate, dedicated, and reliable LPNs and RNs to join our nursing team in a long-term care/skilled nursing setting. The ideal candidate will provide high-quality care to residents, ensuring their safety, comfort, and well-being in accordance with established policies, procedures, and regulations.
Key Responsibilities:

Deliver direct nursing care to residents under the supervision of the DON or Unit Manager.

Administer medications and treatments as prescribed by physicians.

Monitor residents for changes in condition and promptly report concerns to appropriate staff.

Document resident care accurately and timely in electronic health records (EHR).

Assist with admissions, discharges, and transfers.

Collaborate with interdisciplinary team members to develop and implement care plans.

Supervise and support CNAs and other nursing staff.

Maintain compliance with state and federal regulations, including HIPAA, infection control, and safety protocols.

Communicate effectively with residents, families, and staff.

Respond promptly to emergencies and follow proper procedures.

Participate in continuing education and facility training sessions.
Requirements
-
LPN: Current valid LPN license in [State].

RN: Current valid RN license in [State].

Graduate of an accredited nursing program.

CPR/BLS certification (required).

Previous LTC/SNF experience preferred but not required.

Knowledge of electronic medical records (EMR) systems (e.g., PointClickCare) preferred.

Strong organizational, communication, and interpersonal skills.

Ability to work flexible shifts, including evenings, weekends, and holidays.
