Job Details

We are seeking compassionate, dedicated, and reliable LPNs and RNs to join our nursing team in a long-term care/skilled nursing setting. The ideal candidate will provide high-quality care to residents, ensuring their safety, comfort, and well-being in accordance with established policies, procedures, and regulations.

 

Key Responsibilities:

  • Deliver direct nursing care to residents under the supervision of the DON or Unit Manager.

  • Administer medications and treatments as prescribed by physicians.

  • Monitor residents for changes in condition and promptly report concerns to appropriate staff.

  • Document resident care accurately and timely in electronic health records (EHR).

  • Assist with admissions, discharges, and transfers.

  • Collaborate with interdisciplinary team members to develop and implement care plans.

  • Supervise and support CNAs and other nursing staff.

  • Maintain compliance with state and federal regulations, including HIPAA, infection control, and safety protocols.

  • Communicate effectively with residents, families, and staff.

  • Respond promptly to emergencies and follow proper procedures.

  • Participate in continuing education and facility training sessions.

Requirements

  • LPN: Current valid LPN license in [State].

  • RN: Current valid RN license in [State].

  • Graduate of an accredited nursing program.

  • CPR/BLS certification (required).

  • Previous LTC/SNF experience preferred but not required.

  • Knowledge of electronic medical records (EMR) systems (e.g., PointClickCare) preferred.

  • Strong organizational, communication, and interpersonal skills.

  • Ability to work flexible shifts, including evenings, weekends, and holidays.
